This is Carle, it is a survey they conduct on recent care/visit. I have experience with these before. It is legit for their customer satisfaction. I have completed them with Dr's visits many times. They use to mail them, with Covid, they now call.

Robocall, claims to be from Carle Hospital, but not a Carle number. Probably a scammer wanting financial, medical, or other personal info.
