Career placement services/employment agency - consumer does NOT recall filling out a "survey" at the end of his job application - Mr. Smith stated that his information was phished/hacked from on line, in some manner, because he had gone directly to the company's job site and then received this unwanted/unauthorized call from WN Positions stating he completed a survey - AFTER his job application/search! He never did this !
