complete this form, do not fill out the accident section this only pertains to physical accidents, however in the space at the bottom note that you have received these faxes from DSM supply
We have been receiving 2 to 4 page faxes two to three times a day from DSM for months now. We have called over 20 times to have our number removed. We have been told every time that our number would be removed. It wasn't. We have faxed their faxes with "Remove our number" on them we still receive their faxes.
I have even blocked their fax phone number, but faxes still come through. I do not believe that they are faxing from the number on their faxes.
Recently I filed a complaint with the Indiana Attorney General against DSM for violating the Telephone Consumer Protection Act. I have not had any response yet from Indiana. Today I investigating the Texas Attorney Generals Website to file a complaint with them.
My suggestion is that everyone file a complaint with the Texas Attorney General, maybe we can get a class action suit going against DSM.

we also have been receiving faxes is there a lawsuit already going on? How would we go about doing this?

I was receiving four pages each day from DSM and finally got it to stop but they sent me some free samples of electrodes and when I said I did not like them they started again - every day four pages of junk. I have used fax blocker on my machine and still persists. The fax number on the page is different from what shows up on my machine. Seems no way to stop it now.

Persistent junk faxes. I've left messages and faxed them to remove me from their fax list and have filed complaints with the Federal Communication Commission. Yet, the junk faxes persist, waisting my fax paper, fax toner and my expensive fax drum.
The only way to stop this is to do it in numbers with the Texas Attorney General and the Dallas TX Distrct Attorney. Go to The Texas Attorney General Website-Consumer Protection-File a Complaint-Telephones & Faxes. You will need the company info which is DSM Supply 11837 Judd CT Suite106E, Dallas, TX 75243, 214-570-0089. Owners name is Scott Malcolm. Once you have done this you can also contact the Dallas Police and file a police report. Once the police report has been processed it will go to the Dallas District Attorney and charges can be filed against him.
