Even though they say they are "Anthem" I do not believe them. I found this from the Anthem website/blog:

Don’t be fooled! This is not about optimal care! This is about their bottom line. Sadly, the ‘Care Team’ is just trying to meddle and gather info.to ultimately keep their costs down. They called on Saturday after denying expensive but essential medicine prescribed by my husband’s doctor the day before.

8:45 PM EDT, caller said he was from Anthem, wanted to tell members about preventive care services. Said I would have to provide personal medical information. I told him no way, I'm not giving personal information over the phone. Send me a secure message through the Anthem site. After the big Anthem hack recently, I assumed this was some scam artist who had access to the hacked personal information, and wanted more personal information.
Received a call from this number. Bianca said she was calling from Anthem BCBS wanted me to know I hadn't maximized my membership and I needed to fill out a survey with important information about myself. I said I worked for Anthem and asked which Anthem she was calling from. She said she didn't know. She said her office was in St. Louis, so she thinks she's BCBS of St. Louis (Anthems are state by state, not by city). I informed her of our policy to not ask for personal information over the phone and she said she really needed it, so I told her she could go through the correct channels and mail the survey to me. Then I hung up.
