medical services to be provided and billed to my medicare card and insurance for a 90 day period, includng house cleaning,toe nail cutting anad doctors comng to my home. . . not my doctor onethey send out from and adress not based in my city. Requwsted my social security, medicare and insurance information. I asked they send me information because do not give this information to callers. That is where I found out that it was a contract that would bill for services at no cost to me.
