I've received about 100 calls to my work number from this number over the past 6 months for a "debt" that was clearly not mine. I had an injury in 2009 and went to physiotherapy for it. The physiotherapist billed my insurance, who didn't pay it for some reason, and now this so-called collection agency has threatened to take me to court over this bill because "you used the service, so you need to pay for it", even though it was covered under my insurance, which the collection agency acknowledged.

I told the woman (I believe her name was Tracy) back in August that I wasn't going to pay for a bill that I'd never even seen, never mind one that wasn't issued to me, and they said that they could mail it to me if I just gave them my address. I refused to provide that information because it's my private information.

"WE ALREADY HAVE YOUR PRIVATE INFORMATION, SIR."
"Then why are you asking for my new address?"
"YOU KNOW WHAT WE MEAN!"
"No, actually, I don't." (I really didn't.)
"Sir, you're being very uncooperative."
"That's because this isn't my problem. The bill was sent to insurance. Insurance didn't pay. This means the matter is between the physiotherapist and insurance. It's not my problem, nor am I going to assume any responsibility for a debt that should have been collected 10 months ago. You guys fight it out."

They refused to accept this, have threatened to take me to court and alter "your file", whatever that is, and have continually called my work number since then.

It may not be common knowledge to everybody, but insurance is paying on your behalf. Should insurance not cover your debt, you are responsible for it and should in fact pay the debt and appeal the decision at the insurance company. There has never been a Tracy working at our agency. Frankly, believing the problem is between the insurance company and the physiotherapist is rediculous.
